My new rewards! For every $60 you spend (before taxes and shipping) you earn a check! After 8 checks, you earn $50 in free products!

• Place an order with me online. Click “shop” on the top of my page (or under menu from a phone)
• The order must be $60 or more BEFORE tax & shipping:
  • A $59.95 order will not qualify for any checked boxes.
  • A $119.95 order will only qualify for one check box. A $120.95 order will qualify for two checked boxes.
  • You cannot combine orders to check a box. Example:
    • One order of $40 and one order of $20.
• Please keep track of your orders.
• Email me the form when you’re done.
• Please allow 2-4 weeks for delivery.
